Prior to Your Initial Appointment

How well do you know your insurance policy? If you’re well versed in the details of your coverage and its limitations, that’s excellent. But if you’re unsure about your dental plan and how it works, this tool will help you understand the benefits available to you.

Due to privacy restrictions, we’re not able to view the details of your dental policy.

At Your Initial Appointment

We ask that you arrive 15 minutes prior to your scheduled appointment time to complete the necessary paperwork. A New Patient Record will be completed in-office. Feel free to download the following forms and complete them prior to your visit. Please remember that direct billing to insurance requires a credit card be left of file for any incidentals. If you have questions, we’re happy to help.

Cancellation Policy

Please note that our clinic requires 24 hours notice to cancel or reschedule existing appointments. In lieu of this notice, a charge of $50 will be incurred.

We recognize that life is unpredictable.

If an emergency arises and keeps you from attending an appointment, we understand. We’ll work with you to rebook as necessary and would be happy to assist you in any way we can.

At the time of booking your appointment(s), we reserve the time especially for you.

When an appointment is missed or cancelled without notice, we are left with an opening in our day that, given proper notice, could have been used for other patients. There are patients who require emergency treatment or those with extenuating circumstances. These patients would benefit from an opportunity to see a dentist as soon as possible. When you give notice, it helps us to help a patient in need.
